Steps

1. 1Activate Phyrexian Altar by sacrificing Cadaver Imp, adding .
2. 2Cadaver Imp dies, triggering Open the Graves and Liliana, Heretical Healer.
3. 3Resolve the Open the Graves trigger, creating a 2/2 Zombie creature token.
4. 4Resolve the Liliana, Heretical Healer trigger, exiling it and returning it to the battlefield transformed, and creating a 2/2 Zombie creature token.
5. 5Activate Liliana, Defiant Necromancer's second loyalty ability by removing three loyalty counters from it.
6. 6Liliana, Defiant Necromancer is put into your graveyard as a state-based action due to having zero loyalty.
7. 7Resolve the Liliana, Defiant Necromancer ability, returning Cadaver Imp from your graveyard to the battlefield.
