Steps

1. 1Activate Altar of Dementia by sacrificing Dark Revenant.
2. 2When Dark Revenant dies, it triggers, putting itself from your graveyard onto the top of your library.
3. 3Resolve the Altar of Dementia ability, causing you to mill cards equal to Dark Revenant's power, which will cause you to mill Dark Revenant.
4. 4As Dark Revenant is a Lhurgoyf due to Maskwood Nexus, Disa's first ability triggers, returning Dark Revenant from your graveyard to the battlefield.
5. 5Repeat.
